We believe that health and wellness are crucial to a student's success both academically and socially. We have a team of dedicated professionals who work together to ensure that our students are healthy, happy, and ready to learn.
Our School Nurse provides medical care, administers medication, and ensures that all students are up-to-date on their vaccinations. Our Social-Emotional Facilitator works with students to develop coping strategies, build self-esteem, and manage stress. Our Resource Team provides support for students and resources for educators so they can meet each student where they are. Through our comprehensive health and wellness program, we aim to promote a healthy lifestyle, prevent illness, and foster personal growth for all of our students.
STUDENT HEALTH INFORMATION
NEW THIS YEAR: FACTS & Magnus Integration
Magnus is our Electronic Health Record (EHR) system, which we use to manage all student health information and health-related communication throughout the school year.
Magnus Health has now been fully integrated with FACTS. This means that parents can now access Magnus through our FACTS Family Portal using a single sign in and password.
To locate Magnus Health in FACTS:
- Login to the FACTS family portal.
- From the left bar, select "Student". This will reveal more options.
- Select the "Medical" option and Magnus Health in bright orange text with an orange login button will be visible.
- Click on the login button, your student/s will immediately appear.

REQUIRED HEALTH FORMS
A NEW STUDENT checklist is included here to assist families with health information requirements for new students.
Immunization Records are required to be provided before a student's first day of attendance by state law. Maryland school immunization requirements can be found here. A computer-generated copy of immunization records is acceptable. If your student has received immunizations in Maryland (excluding military clinics), their records may be accessed free of charge through the Maryland State Health Department here. Please note the following vaccination requirements for specific grade entry:
- Students entering Kindergarten are required to show evidence of 2 doses of Varicella vaccine (VZV/chicken pox) and Measles, Mumps and Rubella, (MMR) to be compliant for school. These are usually given at age 4.
- Students entering 7th Grade must show proof of Tdap (Typhoid, Diphtheria and Pertussis) and Meningococcal conjugate (MCV4) vaccines. These are usually given at age 11.
A School Entry Physical Examination form is required for ALL NEW students. The form must be completed and signed by a healthcare provider and uploaded into Magnus.
A Medication Administration Authorization form is required for each medication, (with the exception of acetaminophen and ibuprofen), if medication needs to be administered to your student during the school day. This requirement applies to both prescription and over-the-counter medications. The form must be completed and signed by both a healthcare provider and a parent/guardian.
An Action Plan is required for all students with anaphylaxis risk (food and environmental/bee stings), asthma, seizures, or diabetes. This form must also be completed and signed by both a healthcare provider and a parent/guardian.

ATHLETICS FORM
Students who wish to participate in the secondary school sports program must also submit a Preparticipation Physical Examination (PPE) form signed by a healthcare provider. This form is separate from the School Entry Physical Examination listed above and must be renewed annually.
Secondary School Athletics
Students in Grades 6-8 are eligible to join our Secondary School Sports programs. (5th grade students are eligible if it is deemed necessary for the team to have more participants, and are invited by the Athletic Director). These practices/trainings generally take place after school, Mondays-Thursdays each season. Information and registration for teams is provided by the Athletic Director.
